Is initial overcorrection after surgical correction for intermittent exotropia necessary?

چکیده
Many pediatric ophthalmologists have been trained with the strategy that an initial postoperative esotropia of 10 to 20 prism diopters following surgical correction for intermittent exotropia is a desirable initial outcome. This recommendation was suggested to apparently reduce the possibility of subsequent recurrence of exotropia. However, pediatric ophthalmologists who do unilateral recession for exotropia with a successful long-term outcome may not always find an initial esotropia following surgery for intermittent exotropia. In this issue, Leow et al. have confirmed that the success rate for surgical correction for intermittent exotropia is not necessary. In fact, they found that most patients with initial overcorrection or orthophoria had a greater degree of subsequent reoccurrence of exotropia compared with patients with initial exotropia. However, the success rate appears to be unaffected by the initial postoperative alignment. Further studies need to be done to evaluate what, if any, desirable initial postoperative alignment following surgical correction for intermittent exotropia results in the best long-term stability of the alignment.

Outcome of Strabismus Surgery and Vision Therapy in a Case of Intermittent Exotropia
Intermittent exotropia is more common and tends to develop later in life. When considering the surgical approach to exotropia, an initial overcorrection may result in better long-term motor alignment [10,11]. Specific recommendations for overcorrection of patients with intermittent exotropia vary: Raab and Parks [12] advised overcorrection by 10∆ to 20∆; Scott and colleagues [13], 4∆ to 14∆; Mc...

PURPOSE To determine the predictive factors that affect short term and long term postoperative drift in intermittent exotropia after bilateral lateral rectus recession and to evaluate its effect on surgical outcome. METHODS Retrospective review of 203 patients with diagnosis of intermittent exotropia, who had surgical corrections with more than 3 years of followup. Different preoperative para...
متن کاملLong-term Surgical Outcomes of Augmented Bilateral Lateral Rectus Recession in Children With Intermittent Exotropia.
PURPOSE To compare the long-term surgical outcomes of augmented bilateral lateral rectus (LR) recession with original surgery. DESIGN Retrospective, nonrandomized clinical study. METHODS A total of 447 children with ≤35 prism diopters (PD) of basic and divergence excess-type intermittent exotropia, who underwent original bilateral LR recession based on the largest angle measured at distance...
متن کاملRefractive error changes in children with intermittent exotropia under overminus lens therapy.
PURPOSE Intermittent exotropia may be decreased by stimulation of accommodative convergence. Once excessive accommodation has been related to myopia, our objective was to evaluate refractive errors changes in children under overcorrecting minus lens therapy. METHODS A retrospective chart review of 21 children with intermittent exotropia was performed. All patients were treated with occlusion,...

PURPOSE To compare long-term surgical outcomes after bilateral lateral rectus recession (BLR) and unilateral lateral rectus recession-medial rectus resection (RR) for the treatment of basic-type intermittent exotropia. METHODS Consecutive patients who underwent BLR or RR for treatment of intermittent exotropia between 1999 and 2010 and underwent ≥5 years of follow-up were recruited for this s...
